Please arrive 10 minutes early for your first appointment to complete a confidential health history questionnaire.
In order to provide safe and effective treatment, each session will include a thorough intake and assessment. Your appointment may include up to 15 minutes of additional time at no extra charge, to allow for orthopedic assessment, functional range of motion testing, and self-care recommendations, if indicated.
A physical/postural assessment may be performed before and/or during your treatment, so please bring loose-fitting, comfortable clothing or athletic wear that will allow you to move freely during your assessment. During the massage treatment, you may undress to your level of comfort, and you will be draped with a sheet and blanket at all times. Following your treatment, we will discuss your treatment plan for ongoing care, and I will recommend self-care hydrotherapy, stretches, exercises, and movement practice that you can perform at home to compliment your treatment and aid with injury rehabilitation.
At the end of each massage treatment, a signed receipt will be issued that you can submit to your insurance provider.
Your appointment time has been reserved specifically for you. In consideration of your therapist and fellow clients, I ask that you provide me with 24 hours notice of cancellation, or a cancellation fee of $50 will be charged for the missed appointment. I understand that life can throw curve balls, so please contact me if you experience a sudden illness or emergency, and I will consider the circumstances and work with you to reschedule your appointment without charges.
Should you arrive late, the treatment will still end at the designated time with no change in fee.
